How to Conduct Blues Health Assessment

Step 1: Log in to your account first.

Only your online account with BCBS gives you access to your health assessment. You can use our website or the Blue Cross mobile app to log in.

Do you need to create an online account? You can sign up on our website or through our mobile app. Remember that any adult completing a health assessment will require their own account.

Step 2: Go to WebMD, then Health & Well-Being.

You’ll be taken to the Blue Cross Health & Well-Being website as a result. Select Resources in the left-side navigation box, then Assess Your Health once you’ve arrived. Select the Health Assessment card from the drop-down menu, then click Take It Now in the pop-up box.

Hints for logging in for Blues Health Assessment

• Before your health evaluation, make sure you can access your online account. Your login and password must be remembered.
• You can restore your username and password online if you forget them. Keep your Blue Cross ID card and the answers to your security questions with you at all times.
